I CAN Prevent Diabetes (SNAP-Ed)

I CAN Prevent Diabetes (Individuals and Communities Acting Now to Prevent Diabetes©) is an educational program designed to help adults diagnosed with pre-Type 2 diabetes prevent or delay the onset of the disease.

Objectives

This program seeks to help adults with prediabetes adopt behaviors to eat healthy foods and engage in physical activity in order to lose weight and prevent diabetes. Participants will learn how to:

  • Set goals for healthier eating and increased physical activity.
  • Take action to meet their goals.
  • Deal with stress and cope with challenges that can slow or derail their progress.

Class sessions include instruction on diabetes prevention and personalized guidance and coaching.

Space and time requirements

This program is delivered weekly for four months (the “core phase”) and then monthly for eight months (the “post-core phase”). Typical sessions are one hour in length. Sessions can be adapted to meet the learning needs of participants, and within reason fit the space and time needs of the partnering agency.

I CAN Prevent Diabetes is a marketing term for the National Diabetes Prevention Program in Minnesota. The program is based on a Centers for Disease Control and Prevention curriculum and National Diabetes Prevention Recognition standards.

University of Minnesota Extension is currently adapting this program for Latino and Somali audiences.
