This accommodation request form has been designed to help families communicate their child’s unique needs to MN 4-H staff to ensure that accommodations are made to serve the youth in the best way possible!
The information collected in this form will be sent to 4-H staff so that they can help volunteers get the proper training and support needed to make accommodations.
The form is available online and should be directed to Jeremy Freeman, 4-H Extension Educator. It will only be shared in confidentiality with MN 4-H volunteers who will be directly involved in serving your child.
