Registration and access (SAHC)

The action required to register for the Systems Approaches for Healthy Communities program vary based on your role within your organization.

  • Are you the lead organizational contact responsible for coordinating the roll-out of the program? You are the Implementation Coordinator.
    See Implementation Coordinator Registration (Package A) below.
  • Are you working with the lead organizational contact to support the coordination and roll-out of the program? You are an Implementation Team Member. See Implementation Team Registration (Package B) below.
  • Are you participating in the program? You are an Individual Participant.
    See Individual Participants Registration (Package C) below.

Please note:

  • The program supports up to 200 individual participants per registered organization.
  • Our preferred way to accept payment is by credit card. If your organization must pay by check, completion of this form is your organization's first step: Direct Billing Contact Form.
  • The University of Minnesota registration system uses pop-up windows, so you may need to disable the pop-up blocker in your browser to successfully complete registration. To test if your computer is ready for this and other issues, see Is my computer ready to take the course?
